

Application Procedures
Submit To:
The Graduate Admissions Office
The completed Graduate School Application Form. (Contact the Admissions
office or Division of Anatomy for Admissions Forms.)
An official transcript from each college or university you have attended,
listing all courses taken, grades and degrees earned, and dates of graduation.
(Request these institutions to send official transcripts directly to the Admissions
Office at Ohio State. Students attending Ohio State should request a transcript
through internal procedures.)
The test score of the TOEFL examination, if required.
Submit To:
The Division of Anatomy
Three letters of recommendation from persons acquainted with your academic
program, scholastic ability, or professional goals and objectives (standard
forms are included in the Graduate School application packet).
A brief, one -to- two page, autobiographical statement describing your
educational and professional goals.
The scores from the Graduate Record Examination (GRE) Aptitude Test
and an Advanced Test in a scientific discipline (scores from an equivalent
national examination may be substituted at the discretion of the Graduate
Studies Committee).
