US/Canada Visiting Students Summary of Requirements

At least 8 WEEKS PRIOR to the start of the rotation, the following must be submitted:

Self-Disclosure Form
Proof of Malpractice Insurance
Certificate of coverage
Proof of Individual Insurance Coverage
Immunization Record (Varicella Titer, Tetanus, Hep B, MMR, current PPD which will cover through final date of rotation at OSU)

At least 4 WEEKS PRIOR to the start of the rotation, the following must be submitted:

Fingerprint Card
will be mailed to you upon receiving application-must be returned to OSU Security in provided envelope.
ACLS Card
proof that the student has completed training
OSU HIPAA Assessment
will be e-mailed to you
Toxicology Screen (10 Panel)-have the results faxed to us at 614-292-0745
