At least 8 WEEKS PRIOR to the start of the rotation, the following must be submitted:

Proof of Malpractice Insurance
Certificate of coverage -
Proof of Individual Insurance Coverage

Immunization Record (Varicella Titer, Tetanus, Hep B, MMR, current PPD which will cover through final date of rotation at OSU)
At least 4 WEEKS PRIOR to the start of the rotation, the following must be submitted:

Fingerprint Card
will be mailed to you upon receiving application-must be returned to OSU Security in provided envelope. -
ACLS Card
proof that the student has completed training -
OSU HIPAA Assessment
will be e-mailed to you -
Toxicology Screen (10 Panel)-have the results faxed to us at 614-292-0745
