Visiting Osteopathic Medical Student applications are accepted from students in good standing who are pursuing a D.O. degree from a school accredited by the Education Department of the American Osteopathic Association (AOA).
All applications must be received at least 8 weeks prior to the start of the rotation you are applying for. Applications for the 2012-2013 academic year will be accepted according to the following schedule:
- Applications will be accepted beginning March 1st, 2012.
- Notification of accepted electives will be sent beginning May 1, 2012.
Faxed applications are not accepted. Incomplete applications will not be processed.
At least 8 WEEKS PRIOR to the start of the rotation, submit the following:
- Application Form (Submission implies consent to a criminal background search)
- $75.00 application fee: Payable to Ohio State University; non-refundable
- Self-Disclosure Form
- Immunization Form (Varicella Titer, Tetanus, Hep B, MMR, current PPD which will cover through final date of rotation at OSU)
Within 1 week of the date your acceptance notification is sent, you must confirm your intent to participate in the available rotation.
At least 4 WEEKS PRIOR to the start of the rotation, the following must be received:
- Copy of ACLS Card (proof that the student has completed training)
- OSU HIPAA Assessment: E-mailed to you in acceptance notification
- Toxicology Screen (10 Panel): results faxed to 614-292-0745
- THC
- Cocaine and Metabolites
- Amphetamine/Methamphetamine
- Benzodiazepines
- Opiates
- Methadone
- Methaqualone
- PCP
- Propoxyphene
- Barbiturates
