Lauren Kirk, PharmD, BCACP is a clinical pharmacist at The Ohio State University Division of General Internal Medicine clinics, currently practicing at Stoneridge GIM in Dublin, Ohio. She received her Bachelor of Science from Xavier University, Doctor of Pharmacy degree from The Ohio State University College of Pharmacy and she completed a PGY1 Pharmacy Practice Residency with a primary care focus. She is a Board Certified Ambulatory Care Specialist (BCACP).
Dr. Kirk believes patients should obtain the best evidence based health care and advice on their medications. Assuring patients understand and are able to follow the providers agreed upon plan without barriers to their medications is a priority. She assists by providing pharmacy clinical services collaborating with physicians and nurse practitioners to design therapeutic monitoring plans for patients. She performs comprehensive medication reviews, works with patients to reach goals for their diabetes and other chronic disease states, and helps resolve medication related challenges. Dr. Kirk also shares responsibilities with the team of GIM pharmacists to provide transitional care management and population health management across OSU GIM practice sites.
