Research Projects
Utilizing QI methodology, we aim to increase appropriate PCN allergy documentation and evaluation through the creation of a PCN allergy documentation workflow. Restructuring of the IHIS documentation activity and creation of a clinical decision support (CDS) tool for PCN allergy evaluation/management will integrate a risk stratification tool to help identify those patients who may benefit from expedited evaluation or dose challenge in an effort to safely de-label these patients as PCN allergic.
Through the triggering of a CDS tool, the risk results allow the care team to determine if the patient is eligible for evaluation or challenge and act on this decision through the associated order sets. A multi-pronged approach will be used to prevent re-labeling of allergy including patient education and EMR alerts.
Educational tools are distributed across the pilot areas prior to implementation. Baseline and ongoing data is collected/analyzed. Adjustments to the system following the PDSA methodology are made. Follow up data is collected/analyzed. Summary reports compiled. Decision on expansion considered.
