Our office no longer accepts email or faxed requests, nor do we complete Medical Board Licensure Forms or Detailed Training Questionnaires.

You can expect to receive an email from MedHub Notification containing a unique/personalized link to access the completed verification letter within 5-7 business days of your submitted request. If you have any questions or do not receive a response within 10 business days, please send an email inquiry to gme@osumc.edu.
