Research and Collaboration
Research is a core mission of the Department of Emergency Medicine. Our research program is supported by multiple federal and industry grants and spans clinical, basic science and translational research realms.
Clinical Research – Clinical research focus areas include emergency geriatrics, oncology, cardiac arrest, prehospital resuscitation, Emergency Medical Services, acute kidney injury, epidemiology and clinical trial design. Emergency medicine research teams screen for clinical trial subjects at the Emergency Departments of University Hospital and OSU East, which collectively care for over 130,000 patients annually. Our clinical research efforts encompass extensive inter-department and inter-institutional collaborations, including active partnerships with the NIH SIREN and PETAL Networks. Clinical research resources include a 3000 sq. ft. office suite, an ED-based Biosafety Level II laboratory, and access to virtual computing resource.
Basic and Translational Research – Basic and translational science efforts focus on development of stem cell therapy and elicitation of the role of non-coding RNAs in acute heart failure recovery. Our basic science resources include laboratories (>1500 sq ft, located in DHLRI and IBMR buildings). The laboratory facilities include multi electrode array (MEA) to assess electrophysiology in human iPSC-derived cardiomyocytes, rodent survival surgery, real-time PCR, controlled hypoxia chamber, isolated perfused heart set-up, plate reader, ultra-centrifuge and BSL2 biosafety hoods.
Medical Education Research – The Department of Emergency Medicine has a strong track record of medical education research on diverse topics. We are ranked #1 among OSU academic departments in the production of peer-reviewed publications related to academic medicine and medical education. Active areas of research include use of virtual reality simulation for training, developing expertise in best evidence procedures education, education methods for improving patient communication, best evidence methods for monitoring and enhancing healthcare provider well-being, and developing methods for combating the effects of systemic racism across the academic health system. Department educators also have a successful track record for mentoring residents and medical students in their pursuit of medical education scholarship.
Ultrasound Research – Ultrasound is another area where The Ohio State has a long tradition of innovation and research. The robust undergraduate and graduate level ultrasound curriculum has allowed our department to pioneer important work in describing novel ultrasound educational techniques including near-peer education, gamification, and virtual learning. Using advanced clinical ultrasound simulators, as well as human Trained Ultrasound Patients, we have been able to study and advance the learning and teaching of ultrasound in shock, transthoracic echocardiography, and transesophageal echocardiography. With new technology, there is now continued focus on how to use handheld devices and artificial intelligence for optimal ultrasound education.
Research Training – The Department sponsors a research fellowship for Emergency Medicine residency trained physicians. The Department also has a research longitudinal track for interested residents.
Center for Serological Testing to Improve Outcomes from Pandemic COVID-19 (STOP-COVID) – Supported by a five-year, $10 million grant from the National Cancer Institute in the National Institutes of Health, is bringing together an interdisciplinary team to examine the long-term effect of the pandemic on first responders, health care workers, and members of the general population. Urine Antimicrobial Proteins in Older Adults: Aging, Infection and Innate Immunity (AMP) – Urinary tract infection (UTI) is the most common bacterial infection in older adults, responsible for 100,000 annual hospitalizations and over $3.5 billion in healthcare costs annually in the US. We will improve diagnosis and management of UTI in older adults by studying the innate immune response of the urinary tract. Antimicrobial proteins (peptides) (AMPs) are key effectors of urinary innate immunity. Preliminary data indicates that several urinary AMPs, including human neutrophil peptide 1-3 (HNP 1- 3), alpha-defensin 5 (HD5), human beta defensin-2 (hBD-2), and cathelicidin (LL-37) can identify UTI in older adults. This could lead to more accurate UTI diagnosis with improvements in clinical outcome. The long-term goal of this proposal is to use the innate immune system, specifically AMPs, to improve the identification, diagnosis, and ultimately treatment of older adults with UTI. Improving Diagnostic Accuracy for Older Emergency Department Patients with Suspected Pneumonia: The Role of Diagnostic Criteria and Novel Antimicrobial Peptides (DAMP) – Pneumonia is currently diagnosed in over half a million U.S. emergency department (ED) visits by older adults each year with an ICU admission rate of 16% and 30-day mortality as high as 16%. Improving emergency department pneumonia diagnosis accuracy in older adults is a priority. Antimicrobial peptides (AMPs) are key effectors of the innate immune system which increase with infection and can be excreted into the bloodstream in pneumonia. Our preliminary data suggests that two AMPs, Human-Defensin 5 (HD5) and Human-Defensin-2 (HBD2), are increased in dyspneic older ED adults with pneumonia. This project seeks to (Aim #1) examine existing pneumonia risk stratification tools for their applicability to the ED setting alone and in combination with antimicrobial peptides to improve diagnostic accuracy. Diagnostic Uncertainty of Infectious Pneumonia Among Patients With Cancer Presenting to the Emergency Department (CAMP) – There are approximately 4.5 million yearly visits to US emergency departments (EDs) by patients with cancer. Pneumonia is the most common ED diagnosis (4.5%, ~202,500 annual visits) in this population lead to admission in 89% of these patients. Pneumonia is associated with significant morbidity, mortality, and healthcare associated costs. Due to the prevalence and severity of infection with a high associated admission rate, correct ED diagnosis of pneumonia is critical. Diagnostic uncertainty and inaccuracy in distinguishing pneumonia from other causes of dyspnea remain a key challenge in the appropriate treatments of patients. This study aims to identify new diagnostic markers to improve ED diagnostic accuracy for infectious pneumonia in patients with cancer.
Implementation of Multidisciplinary Assessments for Geriatric Patients in an ED Observation Unit (IMAGE) – Multidisciplinary assessment by geriatricians, physical therapists, case managers, and pharmacists identifies and addresses underlying geriatric issues in older ED patients. We have developed a two-step protocol to address these barriers: Step 1 is ED nurses using quick, sensitive screens for fall risk (4 Stage Balance Test), delirium (Brief Delirium Triage Screen), and frailty (Identifying Seniors at Risk Score). Patients with concerning results will be placed in an ED Observation Unit for (step 2) multidisciplinary geriatric assessment. In Aim 1 we will use the Consolidated Framework for Implementation Research and Lean Six Sigma methods to identify and address residual barriers to full implementation. Aim 2 will evaluate the effectiveness of this protocol in regards to patient-oriented outcomes (functional status and health-related quality of life at 90 days). Prevention and Early Treatment of Acute Lung Injury (PETAL) – The Clinical Trials Network for PETAL is a network of 12 clinical centers and one clinical coordinating center funded by the NIH National Heart, Lung and Blood Institute to develop and conduct randomized controlled trials in acute lung injury (ALI) and acute respiratory distress syndrome (ARDS). Crystalloid Liberal or Vasopressors Early Resuscitation in Sepsis (CLOVERS) – Sponsored by the NIH PETAL Network, CLOVERS is a multi-center, prospective, phase 3 randomized trial to determine the impact of a restrictive fluids strategy (vasopressors first followed by rescue fluids) as compared to a liberal fluid strategy (fluids first followed by rescue vasopressors) on 90-day in-hospital mortality in patients with sepsis-induced hypotension. Strategies to Innovate Emergency Care Clinical Trials Network (SIREN) – SIREN is a clinical trials network funded by the National Institute for Neurological Disorders and Stroke (NINDS), the National Heart Lung and Blood Institute (NHLBI) and the National Center for Advancing Translational Science (NCATS). The goal of the SIREN Network is to improve the outcomes of patients with neurologic, cardiac, respiratory and hematologic emergencies by identifying effective treatments given in the earliest stages of care. OSU is a member of the Oregon Health and Science University SIREN Hub. (Site PI: Caterino/Wang. Funding Source: NINDS)
Influence of Cooling Duration on Efficacy in Cardiac Arrest Patients (ICECAP) – Neurological death and disability are common outcomes in survivors of cardiac arrest. Therapeutic cooling of comatose patients resuscitated from shockable rhythms markedly increases the rate of good neurological outcome. In this trial led by the NIH SIREN Network, ICECAP will randomize comatose adult survivors of out of hospital cardiac arrest to varying durations of therapeutic hypothermia. Hyperbaric Oxygen Brain Injury Treatment Trial (HOBIT) – Preclinical and clinical investigations indicate that hyperbaric oxygen (HBO2) has a positive impact on reducing brain injury and improving outcomes in severe TBI. By markedly increasing oxygen (O2) delivery to the traumatized brain, HBO2 can reverse the lack of O2 that precipitates cellular energy failure and subsequent brain cell death. Led by the NIH SIREN Network, HOBIT is an adaptive clinical trial designed to determine the combination of HBO2 treatment parameters that is most likely to demonstrate improvement in the outcome of severe TBI patients in a subsequent phase III trial. Brain Oxygen Optimization in Severe TBI Phase-3 (BOOST-3) – Led by the NIH SIREN Network, BOOST3 is a randomized clinical trial to determine the comparative effectiveness of two strategies for monitoring and treating patients with traumatic brain injury (TBI) in the intensive care unit (ICU). The study will determine the safety and efficacy of a strategy guided by treatment goals based on both intracranial pressure (ICP) and brain tissue oxygen (PbtO2) as compared to a strategy guided by treatment goals based on ICP monitoring alone. End-Tidal Carbon Dioxide to Drive Personalized Resuscitation from Cardiac Arrest – Nearly half a million Americans suffer from out-of-hospital cardiac arrest (OHCA) annually, with only 10% surviving to hospital discharge. End-tidal carbon dioxide (ETCO2) measurement is a tool that is widely recognized, easy to use and can potentially provide real-time insights of ongoing resuscitation efforts. The depth of information in continuous ETCO2 capnography in cardiac arrest resuscitation has not yet fully been evaluated; therefore, it has yet to be leveraged in personalized resuscitative care. We are actively characterizing ETCO2 capnography dynamics during OHCA resuscitation to determine its potential benefit in guiding personalized resuscitation. (PI: Nassal)
Pragmatic Airway Resuscitation Trial (PART) – Resuscitation from out-of-hospital cardiac arrest (OHCA) requires optimal delivery of oxygen to the lungs for circulation to the body. The PART trial tested the effect of initial endotracheal intubation vs. initial laryngeal tube airway management upon outcomes in adult OHCA. (PI: Wang, Funding Source: NHLBI)
Prehospital Airway Control in Trauma (PACT) Trial – Airway management is essential in the care of critically ill trauma patients. The PACT trial of the Department of Defense LITES network is carrying out this multicenter stepped-wedge trial in 15 cities to determine if a prehospital strategy of i-gel airway management results in better outcomes than with standard advanced airway management measures in adults suffering from major trauma. REGARDS-Sepsis – Current clinical initiatives focus on the acute resuscitation and care of individuals suffering from sepsis. However, very little information exist to identify the individuals most vulnerable to this life-threatening condition. The NIH-funded REasons for Geographic And Racial Differences in Stroke (REGARDS) study is one of the largest ongoing national cohort studies of community-dwelling individuals in the US. The objective of the REGARDS-Sepsis is to determine the clinical and sociodemographic factors associated with 10-year risk of sepsis in the REGARDS cohort. (PI: Wang, Funding Source: NINR)
REGARDS-Acute Kidney Injury – Acute kidney injury (AKI) is common in hospitalized patients and strongly associated with greater resource utilization, morbidity, and mortality. It is not clear why some patients may experience AKI events when subjected to certain kidney stressors while others do not. This study will determine if novel biomarkers of renal tubule dysfunction are associated with long-term risk of AKI after sepsis, heart failure and cardiac surgery. (MPI: Wang, Funding Source: NIDDK)
Identifying Genetic Markers Associated With Opioid Use And Opioid Use Disorder (PGX) – In collaboration with the University of Cincinnati and The Ohio Attorney General’s Office, this study aims to report the prevalence of genetic markers associated with opioid use disorder within a general emergency department patient population. (Site PI: Bischof, Funding Source: The Ohio Attorney General’s Office)
Cardiac MRI Strategy for the Management of Patients with Acute Chest Pain and Detectable to Elevated Troponin (CMR-IMPACT): A randomized clinical study funded by the NIH National Heart Lung and Blood Institute and led by Dr. Jeffrey Caterino at OSU in collaboration with Dr. Chadwick Miller (principal investigator) from Wake Forest University Health Sciences. This study aims to determine the optimum management strategy for patients with intermediate to high-risk acute chest pain. A cardiac MRI-guided strategy will be tested as an alternative to invasive management strategies in detecting significant coronary disease and the need for coronary revascularization.
Care of Older Adults with Fragility Fractures in the ED (COFFEe): COFFEe is a pilot study led by Dr. Lauren Southerland at OSU to identify and prevent ongoing morbidity and maintain independence after fragility fractures in older adults.
Expanding Antimicrobial Stewardship: A prospective, observational cohort study funded by the NIH National Institute on Aging to develop an antimicrobial stewardship program based on the use of health information technology for patients presenting to the emergency department from a long-term care facility. This study aims to improve diagnostic accuracy and correct prescribing of antimicrobials. (PI: Caterino, Funding Source: NIA)
Improving Syncope Risk Stratification in Older Adults: A multi-center, prospective, observational cohort study funded by the NIH National Heart Lung and Blood Institute and led by Dr. Jeffrey Caterino at OSU in collaboration with Dr. Benjamin Sun (principal investigator) from Oregon Health and Science University. This study gathers information for the development of a novel risk-based predication model to increase clinical benefit and reduce cost compared to existing care of older adults who present to the emergency department with syncope.
Measuring the Effect of Ohio's Geriatric Trauma Triage Criteria: Using the Ohio Trauma Registry and funded by the Ohio Department of Public Safety, Dr. Jeffrey Caterino is investigating the effects of the Ohio Geriatric Trauma Triage Criteria on outcomes in injured older adults.
Neurological Emergencies Treatment Trials Network (NETT): NETT was created by the NIH to conduct large trials to reduce the burden of very acute injuries and illnesses affecting the nervous system from a variety of pathologies ranging from stroke to traumatic brain injury, to seizures to meningitis. Ohio State's Department of Emergency Medicine participates in NETT in collaboration with the Department of Neurology and Dr. Michel Torbey (OSU NETT PI). Procalcitonin Antibiotic Consensus Trial (ProACT): A randomized, multi-center study funded by the NIH National Institutes of General Medical Sciences and led by Dr. Lauren Southerland at OSU in collaboration with Dr. David Huang (principal investigator) from the University of Pittsburgh. This study is designed to investigate the effect of implementing a novel procalcitonin guideline on antibiotic use and adverse outcomes in emergency department patients with lower-respiratory tract infections.
StrokeNet: StrokeNet was created by the NIH to conduct small and large clinical trials and research studies to advance acute stroke treatment, stroke prevention and recovery and rehabilitation following a stroke. Ohio State's Department of Emergency Medicine participates in StrokeNet in collaboration with the Department of Neurology and Dr. Michel Torbey (OSU StrokeNet PI).
