Improving patient outcomes
The Division of Gastroenterology, Hepatology and Nutrition at The Ohio State University Wexner Medical Center is interested in helping primary care physicians with the management of common gastrointestinal diseases. We have developed a “fast-track” methodology for those "high value care" diseases and symptoms in accordance with the American College of Physicians (ACP), American Gastroenterological Association (AGA) and American Association for the Study of Liver Disease (AASLD).
This initiative by the ACP is designed to enable more effective and patient-centered communication between the primary care physician and gastroenterologist. The High Value Care Program facilitates clear communication between primary care and sub-specialist practices so that doctors can provide seamless, coordinated and quality care to their patients.
The conditions chosen by the AGA, AASLD and ACP to include as part of the High Value Care Program include:
- Gastroesophageal reflux disease
- Chronic diarrhea
- Rectal bleeding
- Abdominal pain
- Hepatitis C
- Abnormal liver function tests
All High Value Care referrals will be handled in collaboration with a staff gastroenterologist or nurse practitioner on the day of referral. You will be given the name of the Ohio State gastroenterologist that will help facilitate the care of your patient and communicate the plan and recommendations.
